请教PI版主关于浪涌问题

我用top223做了5v/3a的一个电源,输入有用一级共模滤波.输出有正负脉冲(0.5v左右),恢复时间为50uS左右.偶尔有电压掉至0V.请问是什么原因造成的,如何改进.谢谢.
